Position: Nurse Practitioners

At the South West Community Care Access Centre (South West CCAC), we support people of all ages and cultures who are facing the challenges of age, illness, injury or disability. The South West CCAC serves nearly one million people living in Grey, Bruce, Perth, Huron, Middlesex, Elgin, Oxford and part of Norfolk Counties. A vibrant young organization, we are committed to providing
client-driven care, working in partnership with our providers and clients.

As a Nurse Practitioner with the South West CCAC, you will play a pivotal role in the future of community care in our region and across Ontario.

You will:
1. Provide care for acutely ill, chronically complex and end of life clients in their homes
2. Be a key member of the client services team, helping clients to live safely in their homes and avoid hospitalization
3. Liaise with clients, families and all health care providers, including family physicians and case managers
4. Use your advanced clinical assessment, monitoring and management skills to provide the best possible client care
5. Receive a competitive salary

The ideal candidate for this role:
• Is a PHC NP and RN in extended class
• Has a valid driver’s license
• Is willing to work flexible hours as required
